How to rub TBQ the wrong way

New Pt: [comes back five min later] Here you go.
TBQ: [checks] Oh, you didn't fill out the second sheet. Do you have insurance?
NP: Yes.
TBQ: Okay, then I'll need you to fill out the top part of this second sheet and then sign here and here.
NP: [glances at the sheet] That's the same information
TBQ: Some is very similar, yes, but this is the insurance form that we need you to fill out so we can process your claim. Also in addition to your name and address, it asks for your insurance information which we also need.
NP: [hands over a post-it with his wife's SSN on it] Here. It's under my wife's name, with [Name of insurance] insurance.
TBQ: Okay, so you can fill that in -
NP: [signs his name in the two spots] I don't want to. Tell you what, I trust you, you can fill it out for me.
TBQ: [stares at him from the mounds of paperwork she's already processing] Oh, can I? Thanks.
[eyeroll]
